Diabetes Care Specialists/RPM Program

We are expanding our services and will be providing patient monitoring for our diabetic patients. This job requires you to be onsite, at the office, every day.

Qualifications:

Must have excellent and clear communication skills, especially interacting with elderly patients

Must be skilled and experienced with navigating electronic health records

Must have experience interacting positively with physicians and advanced practice providers, with ability to deliver clear and direct patient information and address any needs arising from the monitoring process

Health coaching: 1 year experience (Required)

Must be efficient in multitasking and prioritizing multiple situations happening at once

Responsibilities:

This will require the addition of a detail-oriented, computer-savvy certified diabetic educator, nutritionist, or medical assistant with a good basic understanding of diabetes, who can:

Educate patients in the use of glucose monitoring equipment and proper processes to use to ensure accurate information is being communicated

Act as a consultant for diabetes education

Develop a sense of accountability in patients by conducting behavioral sessions 

Create a healthy lifestyle management plan and provide emotional support

Conduct monthly calls to check in with each patient, go over their glucose readings, check in with them regarding goals that are set by the practitioners, resolve out-of-range readings, and encourage them to achieve better health outcomes

Work with the nurse practitioners to update goals when needed

Monitor patient readings for glucose and weight

Address healthcare concerns and answer questions

Help patients stay on track with health goals

Keep patients up to date with preventive services

Maintain thorough and accurate visit documentation in our EMR
