Position Overview
The Division of Developmental Disabilities is seeking to hire a dynamic and passionate individual to provide leadership and oversight of the developmental disability/ habilitation plan coordinator services at the Choate Mental Health & Developmental Center in Anna. Choate Mental Health and Developmental Center offers a variety of treatment programs and services including but not limited to: Psychiatric/psychological, medical/physical, social, educational, vocational/rehabilitation, recreational, speech, language and hearing, pharmacy, dental and dietary services, and referrals and special consultations for individuals with developmental disabilities or intellectual disabilities.
Job Responsibilities
1.Serves as the Director of Habilitation Services for individuals served at the Choate Mental Health and Developmental Center.
2.Serves as full line supervisor.
3.Coordinates the Habilitation Team and the overall management of the therapeutic milieu.
4.Monitors and approves the development of plans including timelines for the discharge plan.
5.Implements facility policy and procedures on the unit and the development and implementation of the unit structure and procedures.
6.Serves as a member of facility committees, sub-committees, and workgroups.
7.Ensures that legal paperwork is completed and submitted in a timely manner.
8.Serves as Administrator on Duty (AOD) on a rotating basis, exercising full authority after business hours, weekends, and holidays.
9.Performs other duties as assigned or required that are reasonably within the scope of duties enumerated above.
Minimum Qualifications
1.Requires knowledge, skill, and mental development equivalent to completion of four (4) years college with course work in a social or human services field.
2.Requires three (3) years progressively responsible administrative experience in a health or human services organization.
Preferred Qualifications (in priority order)
1.Three (3) years of professional supervisory experience, preferably five (5) years, including assigning work, providing guidance to subordinates, recommending counseling and/or discipline following collective bargaining agreements, training staff and approving time off.
2.Five (5) years of professional experience working in a developmental treatment program.
3.Three (3) years professional experience working with current diagnostic and treatment modalities for individuals with intellectual disabilities and/or developmental disabilities.
4.Two (2) years of professional experience communicating in oral or written form with internal and external stakeholders.
5.Two (2) years of professional and/or managerial experience in a residential treatment facility.
6.Two (2) years of professional experience implementing policies and procedures for a treatment program in a residential setting.
7.Two (2) years of professional experience performing quality assurance assessments, evaluating services to individuals, in adherence to established goals and standards of care.
8.Two (2) years Extensive knowledge understanding the causes, nature and treatment of developmental disabilities and the methods and techniques employed in individual care development and rehabilitation.
Conditions of Employment
1.Requires the ability to work after business hours, weekends, and holidays on a rotation basis.
2.Requires the ability to use common office equipment, including computers and productivity software programs.
3.Requires the ability to meet all agency vaccine/health-related policies and guidance.
4.Requires ability to pass the IDHS background check.
5.Requires ability to pass a drug screen. On January 1, 2020, the Cannabis Regulation and Tax Act made it legal for residents over the age of 21 to produce, consume and sell cannabis in Illinois. Please note that although the law has changed, the Department will continue to enforce the Department’s zero tolerance Drug Testing policy. The use of unauthorized drugs, including cannabis, by an employee, regardless of the position held is prohibited.
